• Personal+, Family+, and Workforce+ memberships are month-to-month and may be cancelled at any time.
• Student+ memberships run for the 9-month academic term.
• Executive+ is an annual membership. Contact us for specific terms.
• Membership fees are billed monthly in advance.
• Memberships cover care at Holy City Med clinics only and do not replace health insurance.
